Please include the required documents. There will be places for you to upload pictures, etc.
Please double check that all your personal information is correct.
Once you submit your application, you will be redirected to PayPal to make your contribution. You do not need a PayPal account to process your payment.
DO NOT SUBMIT THE FORM MULTIPLE TIMES. It does not replace your original application and just confuses the system.

If you do not have an email address, please enter the email address of someone who can contact you. Please verify the email address below. If possible, please state your Membership ID Number or previous branch name.

Your Photo

The photo for your membership card should be from the shoulders up and show your face clearly, taken straight on, preferably on a plain background. Please make sure you photo meets the following requirements:

color photo
face front
no hat
no sunglasses
no bandanas or beanies
no angled photos
no ‘cut off’ foreheads
no other covering or lighting that obscures your photo
Please do not submit a license photo as we are unable to use a watermarked photo for your membership card

Preferred file types are jpg, png, and bmp. This is a legal ID picture similar to those on a driver's license or passport and must appear similar to those.

Tribal Membership card *required for tribal discount* (scan or picture)

Letter of Recommendation from Leader. *required for selected membership*

Copy of DD214 or current CAC *required for Military/Veteran discount* (scan or picture)
